PWDF Comments on Social Security Administration (SSA) Docket SSA-2014-0027, Agency Information Collection Activities: Proposed Request and Comment Request Regarding SSA Forms for Earnings Statements and for Allegations of Discrimination by SSA
People With Disabilities Foundation (PWDF) recently submitted public comments to the Social Security Administration (SSA) with recommendations regarding SSA Agency Information Collection Activities: Proposed Request and Comment Request regarding two SSA forms.
PWDF is a § 501(c)(3) nonprofit agency with expertise in medical (psychiatric and/or developmental)-legal issues and bases these comments on its 16‑year history of providing legal representation for Social Security Disability Insurance (SSDI) and Supplemental Security Income (SSI) issues for people with psychiatric and/or developmental disabilities.
Regarding Form 7004 – Request for Social Security Statement, PWDF recommends that the SSA tell all individuals who receive their earnings record to check it to make sure that what is being reported is accurate, and why it is important to check. This is especially important as to the number of quarters of employment and ensuring that earnings are neither under-reported nor over-reported.
Regarding Form 437 – Complaint Form for Allegations of Discrimination in Programs or Activities Conducted by the Social Security Administration, PWDF made several recommendations and observations about the use of this form based on PWDF’s experience as legal counsel for two plaintiffs that sued the SSA for equal access to its programs based on mental and/or developmental disability.
